Admissions Checklist

Tour
[   ] Call to sign up for a School Tour and Open House.

Application for Admission
[   ] Complete Application for Admission (both sides) and send with the application fee of $65. 

        Application for Admission deadline is December 8, 2010.
[   ] Please attach a family photograph.
[   ] Please enclose copies of any current reports from specialists and any other educational or

        psychological evaluations you may have.

Records Release
[   ] Sign and send Records Release(s) to your child's current school(s).

Testing
[   ] Grades 3 and above, please call the Educational Records Bureau (ERB), (212) 672-9800, to arrange

        for testing.  Kindergarten - Grade 2 (not required), however, please have ERB send results if tests

       were or will be taken.

Interview Process
Once we receive your application and fee, we will contact you to schedule the following:
